ABSTRACT

Dirofilariasis is a zoonotic disease caused by Dirofilaria immitis, a nematode found mainly in the pulmonary artery and right chambers of the heart, lungs, and large vessels of dogs. This parasitism also occasionally occurs in cats, causing an amicrofilaremic and asymptomatic infection, resulting in severe illness and rapid death. In this case report, it was described acute clinical signs and histopathological alterations in a domestic cat with heartworm disease from the city of Mossoró, the Rio Grande do Norte, Brazil. The nematode species, D. immitis, was confirmed by morphological and molecular analyses. This is the first documented and full report of feline heartworm disease in northeastern Brazil.(AU)


A dirofilariose é uma doença zoonótica causada por Dirofilaria immitis, um nematódeo que parasita cães, principalmente a artéria pulmonar e as câmaras direitas do coração, pulmões e grandes vasos. Este parasita também ocorre ocasionalmente em gatos, geralmente causando uma infecção amicrofilarêmica e assintomática, que pode resultar em doença grave e morte rápida. Neste relato de caso, são apresentados os sinais clínicos agudos e alterações histopatológicas em um gato doméstico do município de Mossoró, Rio Grande do Norte, Brasil, com dirofilariose. A espécie de nematóide, D. immitis, foi confirmada por análises morfológicas e moleculares. Este é o primeiro relato documentado e completo de dirofilariose felina no Nordeste do Brasil.(AU)

ABSTRACT

Abstract Cats are less susceptible to Dirofilaria immitis infection than dogs. Although rare, the feline disease can be fatal even with low parasitic loads. The infection is often asymptomatic or has non-specific symptoms that are mainly associated with the death of immature worms. Microfilaremia is rare and transient. Normally, microfilaremia, when present, lasts for not more than 33 days. This study describes a feline case presenting with non-specific clinical signs and prolonged microfilaremia. Case: a random bred cat infected by feline leukemia virus (FeLV) that was found to be microfilaremic by chance. The infection was detected by the presence of microfilariae in a blood smear and was confirmed by antigen test (SNAP Feline Triple Test, Idexx®) and echocardiogram.


Resumo Gatos são menos susceptíveis à infecção por Dirofilaria immitis do que cães. Apesar de rara, a doença nos gatos pode ser fatal mesmo com baixas cargas parasitárias. Muitas vezes, a doença é assintomática ou apresenta sintomas inespecíficos, principalmente associados com a morte de formas parasitárias imaturas. Microfilaremia é rara e transitória. Normalmente, quando ocorre microfilaremia, ela permanece por, no máximo, 33 dias. Este estudo descreve o caso de um felino que apresentava sinais inespecíficos e microfilaremia prolongada: um gato sem raça definida, portador de infecção pelo vírus da leucemia felina (FeLV) que foi diagnosticado como microfilaremico ao acaso. A infecção foi detectada pela presença de microfilárias em esfregaço sanguíneo e, posteriormente, confirmada pelo teste de antígenos (SNAP Feline Triple Test, Idexx®) e por ecocardiograma.

Abstract Heartworm disease is a health problem for dogs and cats, especially in tropical and subtropical coastal regions of the world. Some studies have compared the efficacy of the diagnostic techniques used to detect this parasitosis. Therefore, the aim of this study was to compare parasitological optical microscopy (POM), serological and molecular techniques for diagnosing canine heartworm infection. Samples were collected between July 2015 and April 2016 from 103 dogs in Cabo Frio, RJ, Brazil. The wet fresh blood, thick smears, thin smears and modified Knott's test were used to detect microfilariae. ELISA (Snap™ 4Dx ® IDEXX) was used to detect antigens and the polymerase chain reaction (PCR) was used to detect DNA and enable sequencing for species differentiation and confirmation. 19.4% of samples were positive according to microscopy. Through PCR, 15.5% of the total were positive. Using ELISA, the positivity rate was 29.1%. Occult heartworm infection was detected in 11.6% of the samples. ELISA sensitivity was shown to be higher than PCR or microscopy (P = 0.001). Sequencing of samples confirmed the presence of Dirofilaria immitis and Acanthocheilonema reconditum . ELISA was more effective for serological diagnosis canine heartworm and should be used in clinical and epidemiological studies.


Resumo A dirofilariose é um problema de saúde para cães e gatos, especialmente nas regiões costeiras tropicais e subtropicais do mundo. Alguns estudos compararam a eficácia das técnicas de diagnóstico usadas para detectar esta parasitose. Portanto, o objetivo deste estudo foi comparar a microscopia óptica (OM), técnicas sorológicas e moleculares para o diagnóstico de infecção por Dirofilaria immitis . Foram coletadas, entre julho de 2015 e abril de 2016, amostras de 103 cães em Cabo Frio, RJ, Brasil. O exame direto, distensão espessa, distensão delgada e o teste de Knott modificado foram usados para detectar microfilárias. O ELISA (Snap ™ 4Dx ® IDEXX) foi usado para detectar antígenos e a reação em cadeia da polimerase (PCR) foi usada para detectar DNA e o sequenciamento para diferenciação e confirmação de espécie. Das amostras, 19,4% foram positivas de acordo com a microscopia. Por PCR, 15,5% do total foram positivos. Utilizando o ELISA, a taxa de positividade foi de 29,1%. Dirofilariose oculta foi detectada em 11,6% das amostras. A sensibilidade ao ELISA mostrou-se superior à PCR ou microscopia (P = 0,001). O sequenciamento das amostras confirmou a presença de Dirofilaria immitis e Acanthocheilonema reconditum . O ELISA foi mais eficaz no diagnóstico sorológico de dirofilariose canina e deve ser usado em estudos clínicos e epidemiológicos.

ABSTRACT

Abstract Dirofilariasis is a little-known zoonosis, with dogs and cats as definitive hosts. It is caused by nematodes and transmitted by mosquito bites. We report the case of a 67-year-old man with a consumptive syndrome with two subpleural pulmonary opacities. A transthoracic lung biopsy revealed a Dirofilaria worm. Myocardial nuclear magnetic resonance (NMR) demonstrated dilated cardiomyopathy after myocarditis related to dirofilariasis. Human infection is rare and occurs accidentally. The most common radiological alteration is a mainly subpleural coin lesion. Dirofilariasis is a neglected emergent disease and knowledge about it is important for differential diagnoses from neoplastic pulmonary nodules.

ABSTRACT

Dirofilaria immitis (Leidy, 1856; Raillet & Henry 1911) is a parasite that is widely disseminated around the globe, with a higher prevalence in warm, humid climates. The first report of its occurrence in South America is from 1878 in Brazil. At that time, reports were scarce and difficult to retrieve ­ therefore, gathering them will facilitate record-keeping over time. Four databases were searched (Scopus, MEDLINE, LILACS, and PubMed) and the search keywords were "Dirofilaria" or "heartworm" and the countries' names. Four countries lacked reports (Bolivia, Ecuador, French Guiana, and Uruguay) and other three (Suriname, Guyana, and Paraguay) had only old reports. Chile was the only country in which studies were conducted over time, and no infected dogs were registered. For the other six countries (Mexico, Peru, Colombia, Venezuela, Argentina, and Brazil), reports showed that the infection frequency varied over time and with the surveyed area. Therefore, the information indicates that D. immitis is established, and veterinarians must institute preventive programs to optimally care for their patients and protect the health of their families.(AU)


Dirofilaria immitis é um nematoide de ampla distribuição geográfica, que ocorre com maior frequência em áreas quentes e úmidas do planeta. O primeiro registro de sua ocorrência na América do Sul foi realizado em 1878, no Brasil. Naquela época os registros eram poucos e raramente de fácil obtenção, razão pela qual reuni-los facilitará a recuperação da memória ao longo dos anos. Quatro bases de dados (Scopus, MEDLINE, LILACS e PubMed) foram estudadas utilizando-se as palavras-chave "Dirofilaria" ou "heartworm", os nomes dos países da América do Sul e o México. Nenhum registro foi encontrado para quatro países (Bolívia, Equador, Guiana Francesa e Uruguai) e para outros três (Suriname, Guiana e Paraguai) os registros eram antigos. Apenas o Chile é o território onde houve estudos registrados com ausência do parasita. Os outros países (México, Peru, Colômbia, Venezuela, Argentina e Brasil) apresentam registros com frequência variável no tempo ou no espaço. Assim, as informações reunidas indicam que infecções por D. immitis ocorrem na maior parte da América do Sul e no México e que os médicos veterinários devem instituir programas preventivos para garantir cuidados médicos de qualidade aos pacientes e para proteger a saúde destes e de suas famílias.(AU)

ABSTRACT

Canine Heartworm Disease (CHD) is a mosquito-borne disease caused by Dirofilaria immitis. In this study, two mature adult-senior dogs from a non-endemic area to CHD presented clinical signs suggestive to the disease. The first one presented skin lesions, loss of appetite, weakness, pale mucosa membrane, and hyperthermia, whereas the second one presented severe ascites, anorexia and exercise intolerance, lateral decumbency, and marked heart murmurs. Both presented tachypnea, thrombocytopenia, leukocytosis, and microfilaremia. Multiplex-PCR (COI gene) resulted positive to D. immitis research in both cases, confirmed by sequencing, with 98% homology to D. immitis (Gen Bank accession n.AJ537512-1). In addition, both animals have never had any prophylactic treatment to CHD, and no reports about traveling to coastal areas. This study reported two unusual cases of D. immitis infection in non-endemic area from Brazil.


A dirofilariose canina (CHD) é uma doença transmitida por mosquitos e causada por Dirofilaria immitis. No presente estudo, dois cães de idade adulta a idoso de área não endêmica apresentaram sinais clínicos sugestivos da doença. O primeiro apresentou lesões de pele, perda de apetite, fraqueza, mucosas pálidas e hipertermia, enquanto o segundo apresentou severo quadro de ascite, anorexia e intolerância ao exercício, decúbito lateral, e murmúrios cardíacos acentuados. Ambos apresentaram taquipneia, trombocitopenia, leucocitose e microfilaremia. A pesquisa por D. immitis pela multiplex-PCR (COI gene) resultou positiva em ambos os casos, confirmada pelo sequenciamento, com 98% de homologia com D. immitis (Gen Bank n. AJ537512-1). Nenhum dos animais havia sido submetido a tratamento profilático para CHD e não havia relatos de viagens para regiões litorâneas. Assim, o presente estudo reporta dois casos raros de infecção por D. immitis em área brasileira não endêmica para a doença.


La dirofilariosis canina (CHD) es una enfermedad transmitida por mosquitos y causada por Dirofilaria immitis. En este estudio, dos perros de edad adulta a anciano, de área no endémica presentaron signos clínicos de la enfermedad. El primero presentó lesiones en la piel, pérdida del apetito, debilidad, palidez de mucosas e hipertermia, mientras el otro presentó severa ascitis, anorexia e intolerancia al ejercicio, decúbito lateral, y soplos cardíacos acentuados. Ambos presentaron taquipnea, trombocitopenia, leucocitosis y microfilaremia. La investigación de D. immitis por multiplex-PCR (gen COI) resultó positivo en ambos casos, confirmados por la técnica de secuenciación, con 98% de homología con D. immitis (Gen Bank n.AJ537512-1). Ninguno de los animales había sido sometido al tratamiento profiláctico para CHD, y sin relatos de viajes a regiones costeras. El presente estudio reporta dos casos raros de la infección por D. immitis en zona no endémica de Brasil.

ABSTRACT

The emergence of Dirofilarial infections in Asia including Vietnam is a clinically significant threat to the community. We here report a rare case of subcutaneous Dirofilaria repens infection on the posterior thoracic wall in a young woman presenting a painful, itchy, and palpable nodule. The adult worm was identified by mitochondrial cox1 and nuclear ITS-2 sequence determination. The diagnosis was additionally confirmed by 16S rRNA sequencing of the endosymbiont Wolbachia pipientis commonly co-existing with D. repens. This is a rare case of subcutaneous human infection on the posterior thoracic region caused by D. repens.

ABSTRACT

Filariasis affecting animals can rarely cause infections in human beings through the accidental bite of potential vectors. The resulting infection in man, known as zoonotic filariasis occur worldwide. Human dirofilariasis, the most common zoonotic filariasis, is caused by the filarial worm belonging to the genus Dirofilaria. Dirofilarial worms, which are recognized as pathogenic in man can cause nodular lesions in the lung, subcutaneous tissue, peritoneal cavity or eyes. Oral dirofilariasis is extremely rare and only a few cases have been documented. We report an interesting case of dirofilariasis due to Dirofilaria repens involving buccal mucosa in a patient who presented with a facial swelling. The clinical features, diagnostic issues and treatment aspects are discussed. This paper stresses the importance of considering dirofilariasis as differential diagnosis for subcutaneous swelling of the face, especially in areas where it is endemic.

ABSTRACT

Dirofilariasis is a rare disease in humans. We report here a case of a 48-year-old male who was diagnosed with pulmonary dirofilariasis in Korea. On chest radiographs, a coin lesion of 1 cm in diameter was shown. Although it looked like a benign inflammatory nodule, malignancy could not be excluded. So, the nodule was resected by video-assisted thoracic surgery. Pathologically, chronic granulomatous inflammation composed of coagulation necrosis with rim of fibrous tissues and granulations was seen. In the center of the necrotic nodules, a degenerating parasitic organism was found. The parasite had prominent internal cuticular ridges and thick cuticle, a well-developed muscle layer, an intestinal tube, and uterine tubules. The parasite was diagnosed as an immature female worm of Dirofilaria immitis. This is the second reported case of human pulmonary dirofilariasis in Korea.

ABSTRACT

Morphological investigations were conducted on four bodies of dogs who died due to severe clinical symptoms following a massive invasion of cardiac and pulmonary Dirofilaria. The subjects were monitored clinically and diagnosed serologically positive for the Heartworm disease. The necropsy examination of the cardiovascular system (right ventricle and pulmonary artery) revealed the presence of 25 adult parasites in one dog with length ranging between 8 and 33cm. Macroscopically, lesions consistently observed were represented by the right ventricular dilatation and the diffuse wall thickening of the pulmonary artery. Parasitic invasion secondary lesions were present in the lungs, liver and kidneys (cardiac and vascular lesions). The histological examination mainly revealed myocardial injury, vascular (dystrophic), pulmonary (circulatory and inflammatory), hepatic (degenerative) and renal (degenerative and inflammatory) damage.


Investigações morfológicas foram realizadas em quatro cadáveres de cães, mortos em razão de graves sintomas clínicos atribuídos à invasão massiva cardíaca e pulmonar por Dirofilaria immitis. Os indivíduos foram acompanhados sob o ponto de vista clínico, sendo diagnosticados serologicamente positivos para dirofilariose. À necropsia do sistema cardiovascular (ventrículo direito e artéria pulmonar,) revelou-se em um indivíduo a presença de 25 parasitas adultos, com comprimento entre 8-33 centímetros. Macroscopicamente, as lesões observadas foram a dilatação do ventrículo direito e o espessamento difuso da parede da artéria pulmonar. Lesões secundárias à invasão parasitária estavam presentes nos pulmões, no fígado e nos rins (lesões cardíacas e vasculares). O exame histológico revelou, principalmente, lesões do miocárdio, lesões vasculares (distróficos) e pulmonares (circulatórias e inflamatórias), mas também no fígado (degenerativas) e nos rins (degenerativas e inflamatórias).

ABSTRACT

Report of two canine dirofilariosis cases of ectopic location in the state of Rio de Janeiro. This is the first report of erratic migration for this parasitosis in dogs in the state, calling attention to the short period of time between the two cases. The fact that the area is endemic for this parasite, its zoonotic potential and the report of human cases in the state, demonstrates that authorities should be alerted to the control programs of dirofilariosis along with the pathogenic profile of the infections.


Relato de dois casos de dirofilariose canina de localização ectópica no Estado do Rio de Janeiro. Este é o primeiro relato de migração errática para esta parasitose em cães no Estado, e chama a atenção o curto espaço de tempo entre os dois casos. O fato da região ser endêmica para a parasitose, seu caráter zoonótico e o relato de casos humanos no Estado, demonstram que as autoridades competentes devem estar atentas aos programas de controle da dirofilariose assim como no perfil patogênico da infecção.

ABSTRACT

From 2006 to 2010, hospitals in Hanoi treated 10 human patients for dirofilariasis. The worms were collected from parasitic places, and identification of the species was completed by morphology and molecular methods. Ten parasites were recovered either from the conjunctiva (n=9) or subcutaneous tissue (n=1). The parasites were 4.0-12.5 cm in length and 0.5-0.6 mm in width. Morphological observations suggested all parasites as Dirofilaria repens. Three of the 10 parasites (1 from subcutaneous tissue and 2 from eyes) were used for molecular confirmation of the species identification. A portion of the mitochondrial cox1 (461 bp) was amplified and sequenced. Nucleotide and amino acid homologies were 95% and 99-100%, respectively, when compared with D. repens (Italian origin, GenBank AJ271614; DQ358814). This is the first report of eye dirofilariasis and the second report of subcutaneous tissue dirofilariasis due to D. repens in Vietnam.

ABSTRACT

Dirofilariasis is a zoonotic disease caused by Dirofilaria, a parasite of domestic and wild animals. The disease is transmitted by inoculation of mosquitoes infected with the microfilariae during their blood meal. Accidental infection of man results in lung nodule, subcutaneous mass anywhere in the body or ocular lesion that may be subconjunctival or periorbital. The incidence of ocular dirofilariasis is on the rise in several parts of India particularly in Kerala. Here we report a case of ocular dirofilariasis with cellulitis presenting as a periorbital mass.

La dirofilariasis es una patología parasitaria, causada por los nematodos Dirofilaria immitis y D. repens, principalmente. Mientras que D. immitis presenta distribución mundial, D. repens se extiende por el viejo mundo. Ambas especies afectan a caninos y felinos domésticos y salvajes, así como a humanos, por lo que es un problema de salud pública mundial. En este estudio se determinó la prevalencia de D. immitis, en caninos del municipio Sucre, estado Sucre, Venezuela. Se tomaron muestras sanguíneas a 138 caninos domésticos, mayores de seis meses, sin distinción de raza ni sexo, previo consentimiento informado por sus propietarios. El diagnóstico parasitológico de las microfilarias circulantes se realizó por examen directo y método de concentración Knott modificado; las características morfológicas fueron observadas en extendidos teñidos con Giemsa. Se utilizó un estuche comercial, para la captura de antígenos de excreción-secreción de D. immitis. De los 138 caninos estudiados, 12 casos positivos (8,7%) fueron detectados parasitológicamente, mientras que por el estuche comercial se detectaron antígenos en 20 caninos (14,5%), lo que demuestra que existe una dirofilariasis oculta en el 5,8% de los casos evaluados. Uno de los perros microfilarémicos, no presentó antígenos circulantes. La prevalencia de dirofilariasis en el municipio Sucre, estado Sucre fue de 15,2%. Este reporte de dirofilariasis canina en el municipio Sucre, representa un importante aporte en el levantamiento epidemiológico de la enfermedad. Futuros estudios permitirán evaluar el riesgo de infección de las personas que conviven con los perros con dirofilariasis.


Heartworm is a parasitic disease, caused by nematodes Dirofilaria immitis and D. repens, mainly. While D. immitis presents worldwide distribution, D. repens extends the old world. Both species affect canines and felines domestic and wild, as well as human, so it is a global public health problem. In this study we determined the prevalence of D. immitis in dogs in the municipality of Sucre, Sucre state, Venezuela. Blood samples were taken for 138 domestic dogs, over six months, regardless of race or sex, prior informed consent by their owners. The parasitological diagnosis of circulating microfilariae was done by direct examination and concentration method modified Knott, the morphological characteristics were observed in Giemsa-stained smears. We used a commercial kit, for the capture of excretory-secretory antigens of D. immitis. Of the 138 dogs studied, 12 cases (8.7%) were detected parasitologically, whereas the commercial kit antigens were detected in 20 dogs (14.5%), demonstrating that there is an occult infections in 5.8% of the cases. One of the dogs with microfilariae in blood, did not submit antigens circulating. The prevalence of heartworm in the municipality of Sucre, Sucre state was 15.2%. This report of canine heartworm in the municipality of Sucre, represents an important contribution in an epidemiologic survey of the disease. Future studies will assess the risk of infection for people living with dogs with heartworm disease.

ABSTRACT

Heartworm disease is caused by Dirofilaria immitis in dogs and cats. Dirofilariasis is clinically considered very important, because of its severe and life threatening disease in case of even light infection. Cats are infected much less commonly, even in areas which present heavily infected dogs. After clinical examination blood samples were collected from 120 companion and stray adult cats in Ahvaz city. Dirofilaria microfilaria was detected by modified Knott method and its antigen detected by immunochromatography. One [2.5 years old], out of 120 [0.83%] of the studied cats was infected with Dirofilaria immitis microfilaria. Comparison male cat of domestic short hair [DSH] breed. This survey is the first report on isolation of Dirofilaria immitis in cats of Ahvaz city. Heartworm infection is one of the most important parasitic diseases in dogs and an increasing number of cases are reported each year by necropsy in Ahvaz. This study was conducted to determine the prevalence of Dirofilaria immitis infection and to investigate the risk factors related to heartworm disease in urban dogs in this area. Blood samples were collected from 100 dogs from Feb 2007 to Feb 2008. All samples were examined by modified Knott test, direct smear and antigen detection [Immunochromatographic antigen rapid canine D. immitis Ag Test Kit] technique to detect circulating microfilaria and adult antigen of D. immitis, respectively. Of the total 100 dogs, 1% were positive by direct smear, 5% were positive by modified Knott test and 6% were positive by antigen detection. In addition, 1% of positive dogs were determined to have occult D. immitis infections. Dirofilaria immitis was the only canine filarial parasite present in this study. From these three methods, modified Knott test, due to microfilarial identification on concentrated blood had the highest validity. The difference between outdoor and indoor dogs was found significant [P = 0.033], whereas no statistically significant differences were observed between different age groups, sexes and breeds [P>0.05]. The highest heartworm prevalence by modified Knott test was observed in older than 6-year-old dogs [8.3%] followed by 3-6 years [2.7%] and the 6-month to 3 years [3.5%] age groups. The infection was more prevalent in males and short haired breeds. Our results suggest that heartworm treatment and preventive care should be considered in urban dogs of Ahvaz, Iran

ABSTRACT

A dirofilariose é uma zoonose pouco conhecida causada por Dirofilaria spp., nematódeo mais conhecido como verme do coração dos cães (Dirofilaria immitis), parasita do sistema circulatório desses animais, mas que também pode acometer gatos e o ser humano. Sua ocorrência está intimamente ligada à presença de mosquitos vetores (Aedes spp., Anopheles spp., Culex spp.), condições climáticas favoráveis, assim como trânsito entre regiões indenes e endêmicas/epidêmicas. O ser humano pode se infectar com D. immitis (pulmão), Dirofilaria repens (pulmão, subcutâneo) e Dirofilaria tenuis (subcutâneo). A fisiopatologia está intimamente ligada à morte do parasita onde, no cão, pode induzir a obstrução de vasos circulatórios e no ser humano produzir uma lesão nodular com intensa reação inflamatória no parênquima pulmonar com formato de moeda observada nas radiografias. Pode ser diagnosticada pelo exame físico, pela detecção de microfilárias na circulação sangüínea, imunoadsorção enzimático (ELISA), alterações radiográficas, ecocardiografia, ultrassonografia e necropsia. Há riscos no tratamento, sendo a prevenção com a utilização de drogas nos animais o método mais eficaz, principalmente em visitas a áreas endêmicas ou epidêmicas, diminuindo-se, assim, o risco para saúde pública devido à disseminação do parasita.


Dirofilariasis is an unknown zoonosis, caused by Dirofilaria spp, nematodea most known as dog's heartworm (Dirofilaria immitis), which parasites the vascular system of these animals, but infects cats and human beings too. Its occurrence is highly linked to the presence of mosquitoes (Aedes spp., Anopheles spp., Culex spp.), adequate climatic conditions, as well as the transit between infection-free and endemic/epidemic regions. Human beings can be infected by D. immitis (lung), Dirofilaria repens (lung, subcutaneous) and Dirofilaria tenuis (subcutaneous). The physiopathology is highly dependent of the parasite death, inducing the obstruction of the vascular system in dogs and nodular lesions with intense inflammatory reaction in pulmonary parenchyma, like a coin lesion, observed in the radiographs. It can be diagnosed by physical examination, microfilarias detection on the vascular system, enzymatic immunoadsortion (ELISA), radiographic alterations, echocardiography, ultrasonography and necropsy. There are risks on treatment and drug prevention in animals is the most efficient method, mainly in visits to endemic or epidemic areas, decreasing the risk to public health due to the parasite dissemination.
